The Right to Request Deletion
You have the right to request that Sentry delete any of your personal information that we have collected or maintained, subject to certain exceptions. Once we receive and confirm your verifiable consumer request, we will delete (and direct our service providers and contractors to delete) your personal information from our records, unless an exception applies. We may no longer be able to provide certain products or services if you exercise your right to delete.
The Right to Request Correction
You have the right to correct your personal information. Once we receive and confirm your verifiable request we will use reasonable efforts to update your information, unless an exception applies. We will also ask any service providers or contractors to whom we have disclosed inaccurate information to correct it.

Sentry is offering this online payment service (“payment service”) as a convenience option to you, our valued customer (“you” or “your”). Your use of the payment service is subject to these online payment terms and conditions. In addition to these online payment terms and conditions, you should review our Privacy Policy. Our Privacy Policy includes important information about the type of information we collect about you, how we collect that information, and what we do with that information. Finally, our website Terms and Conditions govern your use of the My.DairylandInsurance.com website and all related pages, information, databases, materials and services, including without limitation the payment service (collectively, the “site”).
Authentication and authorization for payment service usage
The payment service incorporates commercially reasonable methods to authenticate the information that you have supplied for making a payment through the payment service. You will not be able to submit a payment until the payment service authenticates you based on the valid Sentry policy number and the zip code associated with that policy number (both of which are reflected on the printed bill you receive from Sentry). You will not be allowed to use the payment service to make payments to Sentry until you have read these online payment terms and conditions and have acknowledged doing so by clicking the acceptance box on the payment page.
As the user of the payment service, and upon your acceptance of these online payment terms and conditions, you will have submitted and you authorize Sentry to store your banking account or debit/credit card account (as applicable, your “designated account”) information. You are responsible for any legal, regulatory, or banking penalties and fees that may be assessed for supplying false information for use with the payment service.
Once you have authorized Sentry to store your Designated Account, Sentry may provide this Designated Account as a payment option for future payments with Sentry. If you want to cancel your Designated Account as a future option, please use the website ability to delete Designated Accounts or contact a Customer Service Representative at 800-334-0090.
The payment service may be used for payment on both consumer and commercial accounts.
If the designated account is a business account, then you must be a properly authorized signer on the account.
By accepting these online payment terms and conditions, you authorize Sentry, through the payment service, to initiate a charge to your designated account according to your instructions. Each time you use the payment service to make a payment to Sentry, you will be reauthorizing Sentry to charge your designated account according to your instructions.
By accepting these online payment terms and conditions, you acknowledge that Sentry will send electronic communications regarding payments to the email address you provided in accordance with the payment service.
By accepting these online payment terms and conditions, you acknowledge that Sentry will retain information you provided in connection with the payment service related to you and your designated account in accordance with any applicable laws, rules or regulations, or as Sentry is required to do so to comply with its contractual obligations.
By accepting these online payment terms and conditions, you agree that:
No payment, or any portion of a payment, will be considered paid until Sentry actually receives payment;
If your banking account has insufficient funds on the effective date of the withdrawal, Sentry may (but is not obligated to) attempt a second withdrawal a few days later and any charges for the initial non-payment or second payment will be paid by you. You will be responsible for any late fees on any payment that is delayed as a result of insufficient funds;
If your payment is not honored for any reason by your bank or debit/credit card company, you are still responsible for making your payment and you will be responsible for any associated late fees;
It is not necessary for any officer or employee of Sentry to sign a debit to your account; and
Sentry will incur no liability if your designated account has an insufficient limit or insufficient funds available.
By accepting these online payment terms and conditions, you represent and warrant that:
You are 13 years old or older;
You are using your actual identity and any information you provide is accurate and complete;
You are authorized to make payments using the designated account;
You have read and agreed to our Privacy Policy;
You have read and agreed to our website Terms and Conditions; and
Your use of the payment service will not violate any local, state, national or international laws or regulations.
Additional terms applicable to automatic payments
For automatic payments, the terms and conditions set forth in this section are in addition to, and not in replacement of, the terms and conditions set forth in the remainder of the online payment terms.
You will not be allowed to enroll for the automatic payment feature of the payment service unless you accept these online payment terms. By accepting these online payment terms and enrolling in the automatic payment feature, you authorize Sentry to store and use your Designated Account to initiate recurring variable payments (debits) for amounts due on the enrolled policy(ies). Automatic payments will be initiated on or about the payment date on a policy or the next business day from the payment account identified by you for automatic payments with respect to such policy. These recurring variable payments will continue until the applicable policy terminates or the automatic payment authorization is cancelled by you or Sentry.
You are responsible for making sure there are sufficient funds in your payment account(s) on the applicable payment date(s) and keeping your payment account information up to date. If any premium payment is not honored by the financial institution or card issuer, coverage on the policy for which payment is to be applied may be cancelled or voided for nonpayment of premium, unless alternative payment arrangements have been made at least one (1) business day before the payment date. If the payment is not honored for any reason by the financial institution or card issuer, then you are responsible for making the payment and any associated late or returned payment fees charged by Sentry. For example, if your credit card number changes or your card expires, then you must update your payment account information on the payment service or provide an alternative means to pay your premium at least one (1) business day before the scheduled payment date. Failure to do so could result in a late fee and cancellation of your policy for nonpayment of premium.
This authorization applies to the policy(ies) you have selected to enroll in automatic payments and any extension, renewal, change or reinstatement of such policy(ies). This authorization will remain in effect until you request termination by calling customer care at 800-334-0090 or by logging into My.DairylandInsurance.com at least two (2) business days before the payment due date.
Sentry online payment service
By providing Sentry with the account information for your designated account, you authorize Sentry to follow the payment instructions we received from you through the payment service (your “payment instructions”). By using the payment service and providing payment instructions, you authorize Sentry to initiate a message to your designated account provider to charge your designated account and send those funds to Sentry for payment on your policy or account. If you are paying a bill received from Sentry and such payment is received after the due date, then, regardless of any cause except for our failure to reasonably process your payment in accordance with your payment instructions, you are responsible for any late charges and interest assessed, the same as if you mailed your payment to Sentry and we received it after the due date. This would be true even if you are unable to access the payment system due to problems with the internet, or our servers being temporarily unavailable. Therefore, we recommend that you use the payment service at least four (4) business days before the actual due date. (Business days are Monday through Friday, except for federal banking holidays). If you make a payment in error or feel that a refund is due, please contact Customer Service at 800-334-0090.
Application security
Our Sentry payment service application employs the following controls and measures:
Uses Secure Socket Layer (SSL) 128 bit encryption for all transmissions of consumer data in order to assure privacy.
The password can be changed by the end user at first sign on and subsequently. This further protects the end user from tampering by others.
Limitations of liability and exclusions of warranties
To the extent permitted by applicable law, you agree that the payment services are furnished to you on an "as is" and "as available" basis without any implied or expressed warranty of any kind (including without limitations, the implied warranties of merchantability, fitness for a particular purpose, suitability, security, correctness and non-infringement). None of Sentry, its subsidiaries, affiliates, nor any of their respective officers, directors, employees, contractors, agents, and suppliers (Collectively the "Sentry parties") Make any representation or warranty regarding the payment services, and you accordingly agree that the Sentry parties are not responsible for, and disclaim all liability for, the continued availability, reliability, reliability, accuracy, results or performance of the payment services, the performance of the internet, the downloading compatibility of any materials or software with your computer system, the existence of any virus, worm, malicious code or other disabling device from any source, the unauthorized access to or use of your information by a party other than Sentry, any technical failures (including hardware or software failures), incomplete, scrambled, or delayed computer transmissions, and/or technical inaccuracies, or loss or use of data, as well as unauthorized access of user transmissions by third parties arising our of or related to these terms. No one is authorized to make any warranty on Sentry's behalf, and you cannot rely on any other statement of warranty. Rely on any other statement of warranty. You assume responsibility for the accuracy, appropriateness, and legality of any information you supply to Sentry.
You agree that the Sentry parties are not liable for any consequential, special, indirect, exemplary, or punitive damages. In no event shall the Sentry parties' total liability to you for any damages, losses, fees, and expenses (including attorneys' fees), whether in contract, tort, trespass or otherwise exceed the amount (if any) paid by you to Sentry to purchase the particular products or services which form the basis of your claim, the amounts paid by you to Sentry to purchase products or services in the three (3) months preceding any claim where your claim does not relate to products or services purchased, or $100 if you have not paid any amounts to Sentry. If you live in a state that does not allow the waiver of certain warranties, or limitations or damages waivers described in this section, some of these waivers and limitations may not apply to you. You agree to promptly notify Sentry in writing if you believe you have any claim against the Sentry parties, and, in any event, you agree that any claim not brought within one year of your discovery shall be deemed waived and released.
If you provide Sentry with an e-mail address for information delivery or otherwise request a non-secure delivery of information, you agree not to hold Sentry responsible for any misuse or unauthorized viewing of that information.
Alterations and amendments
These online payment terms and conditions and any policies referenced herein or attached hereto are the legal binding agreement between Sentry and you with respect to the payment service. You will be agreeing to the current provisions of these online payment terms and conditions each time a payment is made using the payment service. These online payment terms and conditions may have been altered or amended from the last time you used the payment service so be sure to read it each time before you make a payment to be certain you still agree with its provisions. Your use of the payment service after any changes to these online payment terms and conditions will constitute your agreement to such change(s).
Termination or discontinuation
We reserve the right to terminate the payment service at any time, or to terminate your use of the payment service. If we terminate your use of the payment service, you will need to pay your sentry bill or premium in another manner.
Disputes
If there is a dispute regarding the payment service, you agree to resolve the dispute in accordance with the dispute resolution procedures and governing law as outlined in the website Terms and Conditions
These online payment terms and conditions, and any policies referenced herein or attached hereto are the complete and exclusive statement of the agreement between you and Sentry regarding the payment service. It supersedes any proposal or prior agreement, oral or written, and any other communications between you and Sentry specifically relating to the payment service. If there is a conflict between what a Sentry employee tells you and the terms of this document, these online payment terms and conditions will prevail.
You should print and retain this document for safekeeping and reference at any time during usage of the payment service.
By clicking the acceptance box on the payment page, you acknowledge that you have read and understand these online payment terms and conditions, the website TERMS AND CONDITIONS, and our PRIVACY POLICY and you agree to be bound by their terms, and any future amendments, which may be made by Sentry from time to time.

Consent
You have the right to receive your billing statements, policy documents, certain legal disclosures, and other items listed below (together, the “Policy Documents”) in paper form from Sentry Insurance Company or its affiliate or subsidiary (collectively referred to as "Sentry Insurance").
By agreeing to this Consent to Electronic Delivery of Policy Documents (“Electronic Delivery Consent”), you voluntarily agree to receive Policy Documents electronically for consumer insurance policies from Sentry Insurance. You understand that by giving us your consent, you may no longer receive Policy Documents in paper form and you accept any consequence of not reviewing the Policy Documents in a timely manner. Your consent will remain in effect until you provide a Withdrawal of Consent notice.
Items Covered by This Electronic Delivery Consent
We will provide electronic Policy Documents, if you provide us with a designated email address and satisfy the system requirements below. The Policy Documents we provide through electronic delivery, include—but are not limited to—your applications and forms, policy declarations, billing statements/cancellation notices, policy endorsements, notices of terms and condition, and other notices, including company notices and notices required by law. However, we reserve the right to provide Policy Documents in paper form at any time at our discretion, even if you have given us consent to provide them electronically, except in Kentucky. In Nevada, we will provide the documents you choose to receive through electronic delivery.
Methods of Providing Policy Documents
In this document, "provide" means to deliver, make available, send, or notify so that we may send the Policy Documents electronically through your Designated Email Address, including those in PDF format or image formats downloaded from our website. It is your responsibility to review Policy Documents promptly. You acknowledge that the Policy Documents will be deemed delivered by Sentry Insurance, if we deliver the policy to the Designated Email Address. Sentry Insurance does not charge fees for the electronic delivery of Policy Documents.
Designated Email Address
You agree to maintain a valid email address ("Designated Email Address") and to continue to have access to the Internet. Further, you confirm that the email address you provide is your email address, and you want us to use it to provide Policy Documents electronically. You agree to maintain each Designated Email Address until you provide us with a new one via your self-service page on My.DairylandInsurance.com after logging in with your username and password, or by calling Customer Service at 800-334-0090.

We do not necessarily retain copies for longer than is required by law. We recommend that you save or print copies of Policy Documents to ensure you have them.
Withdrawal of Consent
You may withdraw your consent for electronic delivery at any time by submitting notice of your request via your self-service page on My.DairylandInsurance.com after logging in with your username and password.
Withdrawing your consent will not affect the enforceability of any Policy Documents you have already received electronically or your obligation to make payments. You understand that it may take up to seven days to process a Withdrawal of Consent request. You may receive electronic notifications in the interim.
Modification of Consent Terms
We reserve the right to modify these terms and conditions at our discretion. We will provide you with notice of such modification(s) electronically in a manner consistent with the section entitled Methods of Providing Policy Documents. If you do not agree to the Consent Statement as modified, you must notify us of your Withdrawal of Consent before its effective date. We will tell you the effective date in the notification. Failure to withdraw your Consent, or take any other required action as stated in the notification before the effective date, will confirm your continued agreement to the Electronic Document Consent as modified. Modifications to these terms will only affect our respective rights and obligations from the effective date of the modification(s) and thereafter or you withdraw your consent.
Security
Email notifications sent by Dairyland® Insurance Services will not contain sensitive or confidential customer information. Due to security risks, you agree that you will not send any sensitive information, such as social security numbers, date of births, or passwords, in an unencrypted email.
Consent and Representations
You have carefully read, fully understand, and agree to be bound by the conditions contained in this Electronic Delivery Consent. You also agree and understand that this choice also applies to any joint account holders and/or policyholders.
